The Wizard of Oz
Released in 1939, The Wizard of Oz would be expected to not have that many convincing special effects. However, the ones that they did include used some pretty dangerous mediums including pyrotechnics, smoke, and fire. Unfortunately for some, this posed a great risk for injury during filming and ended up ruining a scene or two for the popular movie.
When Dorothy first arrives in Munchkin Land and the Wicked Witch of the West appears in a puff of red smoke, she attempts to get the ruby slippers from the young heroine only to be disappointed that she is unable to steal them. Frustrated, she leaves in the same puff of red smoke that she arrived in, frightening the people of Munchkin Land with her intense departure.
In this scene, we can see that the actress that plays the Wicked Witch of the West (Margaret Hamilton) was too slow getting over to her mark. When she went down into the trap door and the fire was set off, she ended up getting first and second-degree burns and was gone from filming for a couple months. She refused to do any more scenes with fire, thus the reason why they kept that take in the movie.
